Superior Dental Group Enrollment

1. You must live in Maryland.
2. You must use a participating provider or you will have no benefits.
3. If you cover your spouse it must actually be your spouse to add to the enrolllment form. If it is your Fiancee or your "live-in" or significant other they must have their own separate policy. No exceptions. It's Maryland law and out of our hands.

4. You can include children under the Two Party or Family Coverage until their 19th birthday. From age 19 to 23 you must submit documentation from school attesting to the fact that they are full time students or they must have their own separate policy. Once a covered child reaches their 19th birthday on the first day of that month they will automatically be dropped from your policy unless you have already submitted proper full-time student documentation in anticipation of their 19th birthday.

5. Your enrollment form must contain the name and telephone number of a participating provider that is currently accepting new patients with the AT94 Fee Schedule. After the first month you can change to another participating provider by calling Member Services at 1-800-720-5948.

6. Proper dental care should be a long term committment so we encourage everyone to anticipate carrying this coverage for 12 consecutive months. However, you do have the right to cancel at anytime for any reason without incurring any kind of penalty.

Limited availability and coverage for Specialists. You receive a 20% discount for work done by participating specialists. To find out what Specialists are available in your area you must call Member Services. Please don't ask us. We don't know.

Your first month cost that is sent in with the completed enrollment form is the monthly cost plus a $25.00 Enrollment Fee:

For just you - $47.00 is mailed with the enrollment form. After that you pay $22.00 monthly.
For Two Party Coverage send $58.50 with the enrollment form. After that you pay $33.50 monthly.
For Family Coverage send $66.00 with the enrollment form. After that you pay $41.00 monthly.

Again, after the first month your cost will be:

For just you - $22.00
For Two Party Coverage - $33.50
For Family Coverage - $41.00

There have been no monthly premium increases in four years. There probably will not be an increase in 2004 but technically the price can be raised any January 1st. If there is an increase and you think it's too high then you can immediately drop out of the program without incurring any kind of penalty.
