A one year MPD memberhip for one person costs $59.00. This will give you access to our network of 150 dentists, General and Specialists, and you pay 20% to 40% less for your work than you would otherwise. This is an excellent program for people who do not have dental insurance since it immediately lowers the cost you pay for a procedure. With traditional dental insurance you don't benefit from the lower cost until the insurance company sends you a reimbursement check. With the MPD dental program you just pay the lower cost at the time of your visit.

During the process of paying by credit card you will be asked for "Billing Information". This will be the name, address, etc. that the CREDIT CARD COMPANY has on record. If the MPD Membership name and address is different than the credit card name and address then complete the "Shipping Information" with the name and address of the person enrolled in our MPD Program.

Shipping Information = Enrollment Information (if different)
