First check below to see if there is a participating dentist convenient to your home or office. A WISE MOVE WOULD BE TO THEN CALL THE PROVIDER TO MAKE SURE THAT THEY ARE CURRENTLY ACCEPTING NEW PATIENTS FROM THE PROGRAM! We STRONGLY suggest that before you submit your enrollment form to us that you call a participating provider and make 100% sure that they are still co-operating with Humana's TDC dental program with the AT94 Fee Schedule. We will not and can not be responsible for you losing the first month or two of your coverage (paying but not have a dentist) because you selected a dentist that is no longer accepting new Humana TDC patients. (It's a hassle and sometimes aggravating but worth it to get these excellent benefits. Please do your homework first.)
Again, you want to call
and ask them if they are currently accepting new
patients with coverage from Humana's T.D.C. (The Dental Concern) program that
have the AT94 Fee Schedule. If the first person you speak
with says, no, then ask to speak with the Office Manager. Remember that if
you do not use a participating provider then you will have NO BENEFITS of
any kind.
The below list of providers is not likely to increase. If there is not a participating
provider on the list that is nearby for you then it is very unlikely that
there will be one in the near future. *

Once you know that there is a provider in your area, then and only then,
should you call us for the actual enrollment form (or
print it out here) although you are always welcome to call at any
time with any questions. ALTHOUGH YOU CAN LATER CHANGE TO ANOTHER PARTICIPATING
PROVIDER, WE CANNOT ACCEPT AN ENROLLMENT FORM LACKING PROVIDER INFORMATION.
YOU MUST WRITE IN YOUR PROVIDER SELECTION BEFORE MAILING YOUR ENROLLMENT FORM.
ON THE ENROLLMENT FORM WHERE IT ASKS FOR "SITE NUMBER" SUBSITUTE
THE PROVIDER'S TELEPHONE NUMBER.

There are very few participating providers left in the Maryland area so it is imperative that you have the name of a provider that is currently accepting new patients in the plan before you submit your enrollment form.
